Ocena potrzeb (Assessment of Need)

Zgodnie z prawem dzieci z niepełnosprawnością poniżej piątego roku życia mogą uzyskać niezależną ocenę swoich potrzeb dotyczących zdrowia i edukacji. Określa to ustawa o niepełnosprawności z 2005 r. (Disability Act 2005).  Każdy miejscowy ośrodek zdrowia dysponuje specjalistą ds. oceny (Assessment Officer). Z miejscowym ośrodkiem zdrowia  można się skontaktować za pośrednictwem HSE:

  • telefonując na infolinię HSE pod numer Callsave (tanie połączenia telefoniczne): 1850 24 1850 w godz. 8.00–20.00 od poniedziałku do soboty;
  • pisząc pod adresem info@hse.ie lub
  • odwiedzając stronę internetową HSE.

Specjalista ds. oceny (Assessment Officer)

Specjalista ds. oceny (Assessment Officer) może:

  • pomóc Wam wypełnić i złożyć wniosek dotyczący dziecka; 
  • zorganizować przeprowadzenie oceny; 
  • zapewnić pomoc i wsparcie podczas procesu oceny oraz
  • przygotować raport z oceny dziecka.

Kiedy ocena potrzeb dziecka (Assessment of Need) będzie gotowa, koordynator ds. oceny potrzeb (Liaison Officer) lub koordynator przebiegu leczenia (Case Manager) przygotują dla Was deklarację usług. Deklaracja usług określi, jakie usługi i wsparcie uzyska Wasze dziecko i od kogo.

By law, all children with a disability who are under five years of age can get an independent assessent of their health and education needs. (The relevant law is the Disability Act 2005). Each Local Health Office has an Assessment Officer. You contact your Local Health Office  through the HSE by:

  • phoning the HSE information line on callsave 1850 24 1850 from 8am to 8pm, Monday to Saturday;
  • emailing info@hse.ie; or
  • visiting the HSE website.

Assessment Officer

The Assessment Officer will:

  • help you with your child's application; 
  • arrange for the assessment to be carried out; 
  • help and support you through the process; and
  • organise your child's assessment report.

When your child’s Assessment of Need is complete, a Liaison Officer or Case Manager will prepare a service statement for you. The service statement will say what services and supports your child will get and from whom.
