Measúnú ar Riachtanais

De réir an dlí, féadfaidh gach leanbh faoi mhíchumas atá faoi bhun cúig bliana d’aois measúnú neamhspleách a fháil ar a riachtanais sláinte agus oideachais. (Is é an dlí ábhartha an tAcht um Míchumas 2005). Tá Oifigeach um Measúnú i ngach Ionad Sláinte Áitiúil. Is féidir leat dul i dteagmháil le d’Ionad Sláinte Áitiúil trí FSS sna bealaí seo a leanas:

  • glao gutháin a chur ar líne faisnéise Fheidhmeannacht na Seirbhíse Sláinte ar ghlao sábhála 1850 24 1850 idir 8am agus 8pm, Luan go hAoine;
  • ríomhphost a chur chuig info@hse.ie; nó
  • cuairt a thabhairt ar shuíomh gréasáin FSS.

Oifigeach um Measúnú

Déanfaidh an tOifigeach um Measúnú an méid seo a leanas:

  • cuidiú le hiarratas do linbh; 
  • socruithe a dhéanamh le go ndéanfar an measúnú; 
  • cúnamh agus tacaíocht a thabhairt duit le linn an phróisis mheasúnaithe; agus
  • tuairisc ar mheasúnú do linbh a eagrú.

Nuair a chomhlánófar Measúnú ar Riachtanais do linbh, cuirfidh Oifigeach Idirchaidrimh nó Cásbhainisteoir ráiteas seirbhíse le chéile duit. Luafar sa ráiteas seirbhíse na seirbhísí agus na tacaíochtaí a gheobhaidh do leanbh agus cé a chuirfidh na seirbhísí agus na tacaíochtaí sin ar fáil.

Is féidir leat sonraí breise a fháil faoin bpróiseas a ghabhann le Measúnú ar Riachtanais ar shuíomh gréasáin an Bhoird um Fhaisnéis do Shaoránaigh.

By law, all children with a disability who are under five years of age can get an independent assessent of their health and education needs. (The relevant law is the Disability Act 2005). Each Local Health Office has an Assessment Officer. You contact your Local Health Office  through the HSE by:

  • phoning the HSE information line on callsave 1850 24 1850 from 8am to 8pm, Monday to Saturday;
  • emailing info@hse.ie; or
  • visiting the HSE website.

Assessment Officer

The Assessment Officer will:

  • help you with your child's application; 
  • arrange for the assessment to be carried out; 
  • help and support you through the process; and
  • organise your child's assessment report.

When your child’s Assessment of Need is complete, a Liaison Officer or Case Manager will prepare a service statement for you. The service statement will say what services and supports your child will get and from whom.
